7/09/2013 New CMS 1500 Claim Form Approved

New CMS 1500 Claim Form Approved On June 18th, the National Uniform Claim Committee (NUCC) announced that the new 02/12 revised 1500 claim form was approved.  This form will replace the current form, version 08/05.

New CMS 1500 Claim Form Approved


On June 18th, the National Uniform Claim Committee* (NUCC) announced that the new 02/12 revised 1500 claim form was approved.  This form will replace the current form, version 08/05.


The NUCC has not yet released the new timeline for use of the new claim form.  However, Medicare anticipates implementing the revised CMS 1500 claim form as follows:

  • January 6, 2014: Medicare begins receiving and processing paper claims submitted on the revised CMS 1500 claim form (version 02/12).
  • January 6 through March 31, 2014: Dual use period during which Medicare continues to receive and process paper claims submitted on the old CMS 1500 claim form (version 08/05).
  • April 1, 2014: Medicare receives and processes paper claims submitted only on the revised CMS 1500 claim form (version 02/12).
These dates are tentative and subject to change.  We will provide more information as it is available.  In the meantime, we are making preparations for the new form.  Should you have any questions, please contact our Customer Service department at 609-265-9200.

*The National Uniform Claim Committee is a voluntary organization that replaced the Uniform Claim Form Task Force in 1995.  The committee was created to develop a standardized data set for use by the non-institutional health care community to transmit claim and encounter information to and from all third-party payers.
